I. DOCUMENT HEADER

1.1 Title. This Affidavit of Service (this “Affidavit”) is made and sworn to as of [DATE] (the “Effective Date”) by [AFFIANT NAME], Oklahoma Licensed Process Server No. [LICENSE NO.], whose business address is [ADDRESS] (the “Affiant”).

1.2 Recitals.

III. OPERATIVE PROVISIONS

3.1 Identity & Qualifications of Affiant.
 (a) Affiant is over eighteen (18) years of age, competent to testify, and not a party to nor interested in the Subject Case.
 (b) Affiant’s Oklahoma process server credentials are current, and Affiant has complied with all bonding and continuing-education requirements mandated by 12 O.S. § 158.1 et seq.

8.1 Governing Law. Any dispute concerning the validity or enforceability of this Affidavit shall be governed by the State Procedural Law of Oklahoma, without regard to conflicts-of-law principles.

8.2 Forum Selection. The District Court presiding over the Subject Case shall have exclusive jurisdiction to resolve any such dispute.

8.3 Arbitration; Jury Waiver; Injunctive Relief. Not applicable to this Affidavit unless expressly ordered by the Court.
